The fall 2025 Leapfrog Hospital Safety grades are out, and four Alabama hospitals scored an "A" grade.
Scores are based on analysis of medical errors, accidents, injuries and infections. Three Alabama hospitals scored an "F."
Among those earning "A" are Flowers Hospital in Dothan, North Baldwin Infirmary in Bay Minette, Baldwin Health in Foley, Riverview Regional Medical Center in Gadsden and Southeast Health Medical Center in Dothan.
Baptist Medical Center South in Montgomery, Huntsville Hospital and Madison Hospital all earned an "F" grade.
Leapfrog has offered safety grades for 20 years. An expert panel uses 32 evidence-based measures to calculate scores. Measures analyzed include process and outcome measures such as infection rates, problems with surgery, safety problems, practices to prevent errors, and doctors, nurses and hospital staff.
Hospitals provide Leapfrog with the safety data. In some cases, that data is not provided or is not available.
Veterans Affairs hospitals, critical access hospitals, specialty hospitals, children's hospitals and outpatient surgery centers are not included.
